  • Age group:
Why are we asking this?: The CDC recommends sexually active people under 26 get tested at least once a year. We, however, do not provide testing service to anyone 18 or under.

  • Gender:

  • Are you pregnant or may be pregnant?:
Why are we asking this?: If you are pregnant, you should see a doctor or health care provider for all of your prenatal testing, including STD testing.

  • If you are pregnant or may be pregnant, please visit your health care provider for all prenatal care and testing, including STD testing.
  • Have you been tested for STDs in the last 12 months?:

  • Which STDs have you been tested for? (select any):

  • Would you like to get tested for these STDs again for confirmation?:

  • Have you had unprotected sex in the last 12 months? (including oral sex):
Why are we asking this?: Sex is all sexual contact. This includes vaginal, oral or anal sex. This also means any fluid exchange (pre-cum, vaginal fluid, blood, or shared sex toys). Condoms and barriers can reduce the risk of infection but they are not perfect.

  • Have you had more than 1 sex partner in the last 12 months?:

  • Are you starting a new relationship?:

  • Were you recently notified that your partner has been diagnosed with an STD?:

  • Which STD has your partner been diagnosed with? (check any):

  • Are you concerned that your partner may be having sexual contact with someone else?:

  • What is this?

It's a risk assessment based on national screening guidelines to help you determine which tests to take and when to take them. The Centers for Disease Control (CDC) and other national health organizations like the American College of Physicians (ACP) have developed screening guidelines for the general population. These guidelines look at factors like age, gender, lifestyle, and risk behaviors to determine if and when routine STD testing should be done.

Getting tested is an important part of taking control of your health. Remember, there are no right or wrong answers. Answering these anonymous questions truthfully and completely is the best first step to better sexual health.
